Getting started with us is easy!

Our goal at Agility Physical Therapy & Sports Performance is to make your initial appointment as easy as possible.  We want to make sure that you start off on the “right foot” for an overall positive experience with us. Customer service is our #1 priority.

Pre-Registration Forms:  To expedite the initial visit process, you may print off and complete any of the following forms prior to your initial visit.  This is recommended if the patient is under 18 years old and their parent/guardian will not be in attendance for the first visit.

Below are some FAQ’s that we can answer before your first visit:

Do I need a physician referral?  Michigan law requires a referral from a physician (MD or DO), physician’s assistant (PA-C), nurse practitioner (NP), podiatrist (DPM), or dentist (DDS) in order to evaluate and treat.  This prescription is needed at the time of your initial appointment. We feel that a collaborative approach is important in your overall injury-healing process, and are happy to work with your personal fitness trainer, chiropractor (DC) or massage therapist to make sure that we are all providing you with the best possible care.

Do you accept my health insurance?  We accept most commercial health insurances, workers compensation and automobile claims.  Our staff will assist you in verifying your benefits and what your financial responsibility would be.  Please refer to our list of insurances Adobe to determine if we are a participating provider.

How do I schedule my first appointment?  To make the process as easy for you as possible, there are several ways to contact us.  Either, contact us through our website and we will call you back within one business day.   Or, simply call our Portage (269.329.0934) or Kalamazoo (269.375.4737) clinic during normal business hours and we will assist you in scheduling your first appointment.  Our goal is to schedule you for your initial evaluation within 24-48 hours.

How long will my appointment last?  Generally, your initial appointment will last approximately 60 minutes, in order for the physical therapist to perform a comprehensive evaluation and get you started down the “road to recovery.”

What should I bring to my first appointment?  Please bring a copy of your insurance card, driver’s license, physician prescription (unless already faxed), and comfortable clothing to be able to access the area that we will be evaluating and treating.
